The following are the new patient packet that must be answered, printed and returned to our office before we schedule a first visit for you. Please review the new patient welcome information.

Print out and complete these forms and mail them to Hawaii Family Physicians PO Box 2060 Kealakekua, HI 96750 or you can drop them off in person at 81-6597 Mamalahoa Hwy. Suite C-201 in Kealakekua along with a copy of your insurance card(s), front and back. After we receive the documents, please call us at 808 323-3107 to set up your first appointment.
