Vaccines / Medication for Outreach to foreign countries

If you want Hawaii Family Physicians to assist you, you must follow this procedure:

Students/Staff

  1. Get your own immunization records in order.
  2. Update all past immunizations on your card including dates given.
  3. Know where you are in any series.  Example: Hepatitis A is a two part series.  (Given in two doses.)    Hepatitis B is a three dosage schedule.
  4. Decide which vaccines or medications you need for the country you are traveling to.  If you want malaria prevention, read about your choices and decide. (link)
  5. Pay school leader for all vaccines, pills, and administration of medication fees two months before outreach begins.
  6. Fill out required paperwork for Hawaii Family Physicians with your permanent address and phone number.  Turn in with your payment.

Notes:

Tetanus must be taken every 10 years.

Typhoid pills cannot be taken simultaneously with malaria pills.  You must start your pill regiment for both of these before you leave.  Typhoid must be taken at least 10 days before you leave for outreach if you plan to also take preventative meds for Malaria  which would then start 2 days before you leave and continue after you return (length of time taking the pills depends on which medication you choose.)

School Leader

1.      Ask students/staff to list all vaccines and medications needed.  (See sample student form)

2.      Contact Hawaii Family Physicians office with the quantity of medications needed to obtain pricing information.

3.      Notify students of prices and collect payments.

4.      Compile your list of names with medications needed and amount paid for each.  (see sample group list)

a.       Organize your list by type of medication.

b.       Include administration fees for each vaccine for our staff to give the injections or handle the prescription.

5.      Turn in your complete list of all medications need along with your check made out to Hawaii Family Physicians two months before outreach so we can place your order.  Vaccines can take two weeks to one month to receive.

6.      Schedule administration appointments with our staff no later than one month before departure.  Make sure students have completed paperwork when they make payment.  Turn in all paperwork.
