| Hawaii Family Physicians offers a clinic once a week on
the campus of the University Of The Nations. The clinic is open on
Wednesdays from 1 pm to 4 pm. For directions, click on the Directions
and Maps above.
All new patients need to fill out paperwork on New Patient page. If you are receiving vaccines only, fill out New Patient Info sheet completely and Privacy Signature page. NOTE: Vaccines are non-refundable once ordered. Vaccines will not be ordered until payment is received. Please allow time for shipping (1-2 weeks minimum) and time for scheduling (2 - 4 weeks). We vaccine no more than 10 staff or students at a time.
